腓肠神经营养血管岛状皮瓣的临床应用

摘　　要：目的报道腓肠神经营养血管岛状皮瓣修复下肢远端软组织缺损的临床应用效果。方法 2003年10月~2009年9月对12例下肢远端软组织缺损骨外露或跟腱外露患者采用腓肠神经营养血管岛状皮瓣修复。创面面积最大6 cm×8 cm,最小3 cm×4 cm。结果 12例皮瓣全部成活,9例伤口一期愈合,2例有浅表感染,1例因骨髓炎皮瓣成活但再次骨外露。随访6~24个月皮瓣质地优良,受区外观满意,小腿三头肌功能无明显影响。结论腓肠神经营养血管岛状皮瓣是修复下肢远端软组织缺损的有效方法。Objective To report the experience of clinical application of distal based sural island flap for the repair of soft tissue defect in the distal lower extremity.Methods From October 2003 to March 2009,the distal based sural island flap was applied to repair heel or foot area in 12 cases.The area of wound ranged from 6cm×8cm to 3cm×4cm.Results All the flaps survived without vascular crisis.Two cases developed superficial infection and one case became bone re-exposed because of osteomyelitis.After a 5 to 6month follow-up,the flap did not damage critical blood vessels and nerves.The operation was simple and the surgery area showed a satisfying appearance.There was no apparent influence as regards the function of leg triceps.Conclusion The flap is a reliable,safe and easy method of providing soft tissue coverage in the distal lower extremity.
