意识障碍病人在气管导管引导下经口腔胃管置入的临床应用

摘　　要：目的探讨意识障碍用传统方法胃管置入困难的病人,在气管导管引导下经口腔置入胃管法的可行性和安全性。方法我科2008年3月～2010年3月随机对136例意识障碍用传统方法胃管置入困难的病人,在麻醉诱导下,采用气管导管引导经口腔进行胃管置入。结果 136例患者均一次置管成功。胃管置入耗时、一次成功率、胃管置入困难人数、胃管置入失败人数、胃管误入气管人数,与同期139例用传统胃管置入法作统计学分析存在差异(P>0.05)。结论在ICU病区中对意识障碍传统方法胃管置入困难的病人,在麻醉医师的协助下采用气管导管引导经口腔胃管置入法是一种一次成功率高,安全可靠的方法,值得在手术室和ICU病区中推广应用。Objective To discuss the feasibility and safety of gastric intubation though mouth under the guidance of endotracheal tube in paitents with disturbance of consciousness.Methods 136 cases with disturbance of consciousness received gastric intubation through mouth under the guidance of endotracheal tube after anesthesia induction.Results The gastric intubation was successfully performed in all the 136 cases with one act;the statistical analysis showed out a significance difference in the time consuming for gastric intubation,one act successful rate,the case number of intubation difficulty,the cases number of intubation failure,the case number of misintubation into tracheal from the conventional gastric intubation（P0.05）.Conclusions Gastric intubation through mouth under the guidance of endotracheal tube is a reliable method with high successful rate for patients with disturbance of consciousness.
