脑挫裂伤后脑组织中AQP4的表达变化及意义

摘　　要：目的观察人脑创伤后挫裂伤脑组织中水通道蛋白4(AQP4)的表达变化,探讨其与脑水肿变化的关系。方法对20例颅脑损伤患者,于伤后2 h～5 d手术时取挫裂伤脑组织和非功能区正常脑叶(脑减压)组织(3例,为正常对照),采用免疫组化法检测所有标本中AQP4,同时观察血脑屏障(BBB)指数及脑水肿的变化。结果颅脑损伤后2 h～5 d,脑挫裂伤区域脑水肿进行性加重,AQP4表达进行性升高,伤后24～72 h达高峰,二者呈正相关(P<0.05);正常脑叶组织中AQP4表达较挫裂伤区各时点均低(P均<0.05);伤后2～24 h AQP4表达与BBB指数呈正相关(P<0.05),余时点无相关性。结论颅脑外伤后脑挫裂伤组织AQP4表达上调,AQP4可能参与其脑水肿的形成过程。Objective To observe the expression of aquaporin 4（AQP4） in contused human brain tissue and discuss the relationship with brain edema.Methods Twenty contusion brain tissues and three lobectomic brain tissues in nonfunctional areas（brain decompression） were obtained from 20 patients who underwent surgery treatment for brain contusions 2 h^5 d after trauma.The expression of AQP4 were detected by immunohistochemistry methods;brain edema and BBB index were observed at the same time.Results Brain edema and the expression of AQP4 increased progressively within 2 h^5 d after brain injury,and reached peak on 24~72 h;they correlated positively each other（P0.05）.The expression of AQP4 in lobectomic brain tissue was lower than that contusion brain tissue at different time points（P0.05）.AQP4 levels correlated positively with BBB index during 2~24 h（P0.05）;and they did not correlate during 24 h^5 d.Conclusions The expression of AQP4 increases in contusion brain tissue after injury.AQP4 may participate in development of brain edema.
