枕颈融合术治疗强直性脊柱炎(AS)患者寰枢椎脱位疗效分析

摘　　要：目的:评价应用Cervifix系统行枕颈融合术治疗强直性脊柱炎患者寰枢椎脱位的临床疗效。方法:回顾性研究2001年4月至2008年6月采用枕颈融合术治疗的5例强直性脊柱炎伴寰枢椎脱位患者的住院及随访资料,分析其麻醉方法,术中注意事项,术后处理及康复效果。结果:5例患者均获随访,所有病例切口一期愈合,植骨融合率100%。无1例发生内固定螺钉及固定棒松动、断裂等并发症。结论:强直性脊柱炎患者具有特殊性,应用Cervifix内固定系统行枕颈融合术,操作方便,固定效果良好,内固定后融合率高。Objectiv： To evaluate the therapeutic effect of occipito - cervical fusion surgery on AS patients with atlantoaxial dislocation by using Cervifix system. Methods：Retrospective research were carried out on 5 AS patients with atlantoaxial dislocation who were given occipitocervical fusion surgery in April 2001 to June 2008. The associated hospitalization and follow - up data were studied and anes- thesia methods, intraoperative cautions, postoperative care and rehabilitation effects were analyzed. Results ： Five cases were all fol- lowed up for six months to eight years （three years i~ average）. The incision in all the five cases was healed in the first stage and graft bones were all fused in December with a bone fusion rate of 100%. There were no complications of loosening and rupture of the internal fixation rods and screws. Conclusion：The AS has its particularity, so the preoperative evaluation, intraoperative anesthesia prevention, nerve injury accident, postoperative rehabilitation and guidance should be noticed. Cervifix internal fixation system is easy to manipulate for AS patients with atlantoaxial dislocation and can achieve better fixation effect with a high rate of fusion.
