冠心病介入诊疗患者焦虑抑郁情绪调查分析

摘　　要：目的研究经皮桡动脉穿刺冠状动脉治疗冠心病患者焦虑抑郁状态及影响因素,为进行心理护理提供依据。方法采用自行设计的一般情况问卷、ZUNG焦虑自评量表(SAS)、抑郁自评量表(SDS)对65例冠心病介入诊疗患者在介入手术前后1～2d进行测评。抑郁自评量表(SDS)、焦虑自评量表(SAS):量表包括20个题目(1、2、3、4级评分),将20个项目评分进行相加,得到总粗分,将总粗分换算成标准分,根据中国常模SDS总粗分的分界值为40分,标准分为50分,标准分50分以上者为阳性。结果 65例冠心病患者经皮桡动脉穿刺冠状动脉介入诊疗前后抑郁自评量表(SDS)标准分为(35.71±9.61)分、(32.34±8.50)分;焦虑自评量表(SAS)分各为(35.35±8.62)分、(31.94±6.48)分,与国内常模比较差异无统计学意义。术前术后抑郁、焦虑发生率各为7.7%、4.6%;6.2%、3.1%。结论经皮桡动脉穿刺冠状动脉介入诊疗前后冠心病患者的焦虑、抑郁情绪的发生率低于以往的研究,对介入手术前后进行心理干预,消除焦虑、抑郁等负性情绪,保证介入手术的顺利进行,具有重要意义。Objective To study the conditions of anxiety and depression in patients after interventional procedures via transradial approach, as well as relevant risk factors, so as to provide theoretical basis for psychological intervention. Methods All 65 patients diagonosed as coronary heart disease are included, ZUNG self rating anxiety scale （ SAS ） and self rating depression scale （ SDS ） are used. Results The SDS scores of patients with the coronary heart disease before and after interventional procedures via yransradial approach were 35.71 ＋ 9.61 and 32.34 ＋ 8.50, respectively. The SAS scores were 35.35 ＋ 8.62 and 31.94 ~ 6.48, respectively, which was not different from domestic regular scores. The incidence of depression and anxiety before and after procedure were 7.7%,4.6%; 6.2%, 3.1%. Conclusion Although the prevalence of anxiety and depression in patients with coronary heart disease before and after coronary artery interventional procedures via transradial approach is lower than previous study. It is important for the performance of operation when patients are treated for anxiety and depression with psychological intervention.
