摘　　要：目的建立一种以疾病为中心,以患者为根本,以社区为核心的新型服务型医疗管理模式。降低慢性呼吸系统疾病急性期发病率,控制并发症,减少死亡率,控制医疗成本。方法通过临床路径的方法对COPD患者不同病程采取社区医疗管理模式进行干预的可行性进行初步探讨和制定临床路径工作表。结果通过临床路径方法建立COPD患者社区医疗管理模式,制定临床路径工作表,可有效降低疾病急性期发病率,控制发作,减少并发症,减少死亡率,有效控制医疗成本。结论 COPD患者社区医疗管理模式临床路径对于改善医患关系,节约医疗成本,提高社区疾病处置水平具有重要意义。Objective To establish a new service-type medical management mode which was characterized by disease-oriented,patient-fundamental,and community-central.So as to reduce the incidence of COPD during acute phase,control the complications,lower mortality,and control the medical cost.Methods By using clinical pathway,the feasibility of intervening COPD patients under different phase in community was discussed,and the clinical pathway work chart was formulated.Results The new management mode was effective to reduce the incidence of COPD during acute phase,control the complications,lower mortality,and control the medical cost.Conclusions The new management mode based on clinical pathway was very important to improve the patient-doctor relationship and save medical cost.
