中西药结合治疗对创伤患者细胞免疫功能的影响

摘　　要：目的:探讨中西药结合治疗对创伤患者细胞免疫功能的影响。方法:60例严重创伤患者随机分为观察组与对照组,每组各30例。两组首先采用常规西药治疗,观察组在此基础上加用扶正祛邪汤口服治疗,比较两组治疗前及治疗后2、46、d白细胞介-素6(IL-6)、C反应蛋白(CRP)水平及外周血T细胞亚群和NK细胞数。结果:两组治疗后2、46、d IL-6和CRP水平均较治疗前有明显差异(P<0.05),观察组IL-6、CRP水平均明显低于对照组(P<0.05)。两组CD3+、CD4+、CD4+/CD8+值及NK细胞数量于治疗2 d后显著降低,治疗后4 d达最低,治疗后6 d开始恢复,与治疗前相比均有统计学意义(P<0.05);观察组治疗后2、46、d CD3+、CD4+、CD4+/CD8+值及NK细胞数量下降较对照组多,但仅在治疗后4 d两组间比较差异有统计学意义(P<0.05)。结论:中西药结合治疗对创伤患者细胞免疫功能的影响显著,能在改善炎症状态的基础上,有效恢复T淋巴细胞亚群和NK细胞的免疫功能,值得临床推广应用。Objective： To investigate the Influence of combined TCM and Western medicine therapy on the cellular immune function of natural killer（NK） and T cells levels in patients with trauma.Methods： Sixty patients with severe trauma were randomly divided into observation group and control group with 30 cases in each group.Both groups were given routine western medicine,the observation group was also given extra herbs for strengthening health and dispelling evil qi.Before and 2,4,6 days after treatment,plasma IL-6 and CRP levels,count of natural killer（NK） and T cells of both groups were observed and compared.Results： Both groups showed significant changes in the plasma IL-6 and CRP levels 2,4,and 6 days after the treatment（P0.05） comparing with that before the treatment.Specifically,the observation group showed significant lower IL-6 and CRP levels comparing with the control group（P both 0.05）.After 2 days of treatment,both groups showed significant decrease in CD3＋、CD4＋、CD4＋/CD8＋ and NK counts,which achieved the lowest level on day 4,and came back on day 6,and were all significantly different from the levels before treatment（P0.05）.On day 2,4,and 6,the observation group showed obvious decrease of CD3＋、CD4＋、CD4＋/CD8＋ and NK counts,but significance was only shown on day 4（P0.05）.Conclusions： Combined TCM and Western medicine therapy can activate the immune function of natural killer（NK） and T cells levels in patients with trauma.
