后腹腔镜肾蒂淋巴管结扎术治疗乳糜尿(附14例报告)

摘　　要：目的探讨后腹腔镜肾蒂淋巴管结扎术在乳糜尿的治疗中的应用价值。方法 2004年7月～2009年6月共收治乳糜尿患者14例,所有患者14例接受经后腹腔镜下肾蒂淋巴管结扎术,双侧患者双侧病变同时结扎。结果所有手术均顺利完成,手术时间60～145min,平均95min,术中出血量约30～120mL,平均50mL,术后未出现严重并发症,术后平均住院6.8d。患者出院时尿液均清亮,出院前复查乳糜试验均阴性。随访3～15个月,无乳糜尿症状复发。结论腹腔镜下肾蒂淋巴管结扎术治疗乳糜尿是一种安全,有效的治疗方法,具有创伤小,并发症少,住院时间短,恢复快等优点,值得临床推广。[Objective] To evaluate the surgical technique and clinical efficacy of retroperitoneoscopic disconnection of renal pedical lymphatic for chyluria.[Methods] We analyzed 14 cases of chyluria.All cases had been treated with retroperitoneoscopic disconnection of renal pedieal lymphatic,the cases with bilateral chyluria were treated with both sides.[Results] All operations were successful.The operation time was 60~145 min,mean 95 min.Blood loss was 30 to 120 mL,mean 50 mL.No severe complications occurred.The average hospital stay after operation was 6.8 days.The urine of all patients was clear after operation,no recurrence of chyluria was found during the follow-up of 3~15 months.[Conclusions] Retroperitoneoscopic disconnection of renal pedieal lymphatic for the treatment of chyluria is a safe,efficient method,with advantages of mini invasion,rapid recovery,less complications and hospital stay,worth to be extended.
