摘 要:目的探讨低风险饮酒对丙泊酚靶控输注(TCI)镇静效应的影响。方法全麻择期手术患者71例,ASAⅠ或Ⅱ级,年龄18-59岁,分为低风险饮酒组(D组,n=38)和非饮酒组(N组,n=33)。术前受试对象均排除相关非处理因素,仅接受丙泊酚TCI镇静,血浆药物浓度设置为3.5μg/ml。观察并记录两组每分钟的HR、SpO2、NIBP、反应熵(RE)、状态熵(SE),以及警觉/镇静(0AA/S)评分、靶效应室药物浓度、停药后靶血浆药物浓度的衰减和不良事件的发生率,OAA/S评分达0分所用时间,OAA/S评分达0分所需丙泊酚用量、血浆靶浓度与效应室靶浓度平衡即刻停药至0AA/S评分恢复为5分所用时间。结果两组患者RE、SE随时间变化趋势相同。OAA/S评分达0分所用时间D组为(402±198)S,N组为(503±255)S,达0分所需丙泊酚用量D组为(2.131±0.663)mg/kg,N组为(2.397±0.746)mg/kg,OAA/s评分恢复为5分所用时间D组为(481±184)s,N组为(521±166)s。结论低风险饮酒的患者对丙泊酚TCI镇静效应无明显影响。Objective To study the influence of low risk alcohol intake on the sedative effects of propofol with TCI (Target-controlled Infusion). Methods Seventy-one ASA I or II patients, aged 18-59 years, scheduled for elective surgery under general anesthesia were enrolled in this study and divided into two g.roups: low risk drinking group(group D,n=38) and non-drinking group(group N, n= 33). All patients were sedated by only propofol before surgery, and the target plasma concentration was 3. 5μg/ml. NIBP(Non-invasive Blood Pressure), HR, SpO2, response entropy (RE), state entropy (SE), the Modified Observer's Assessment of Alertness/Sedation(OAA/S) score, target effect site concentration of propofol, target plasma concentration of propofol after stopping TCI, and the time when OAA/S score was 0, the dosage(mg/kg) of propofol when OAA/S was 0, the time when OAA/S returned to 5 in both groups were recorded. Results There was no difference between two groups in the changes of RE and SE. The time when OAA/S scale reached 0 was(402±198) s in group D and(503 ±255)s in group N; The dosage of propofol when OAA/S scale reached 0 was(2. 131±0. 663)mg/kg in group D and(2. 397±0. 746)mg/kg in group N; The time when OAA/S returned to 5 was(481±184)s in group D and(521±166)s in group D. Conclusion The sedation effects of propofol by TCI have no significant influence in patients with low risk alcohol intake.
