弥漫性轴索损伤患者血清NSE水平的变化及临床意义

摘　　要：目的:探讨弥漫性轴索损伤患者血清NSE水平变化与患者病情严重程度及预后的关系。方法:取2009年1月～2010年6月一共32例急性弥漫性轴索损伤患者为试验组,其中入院时GCS评分9～12分者共13例(A组),GCS评分3～8分共19例(B组),试验组分别于伤后24小时、3天、7天时测定NSE的血清浓度,同时以同期体检中心就诊的患者(均经头颅CT证明无脑实质病变)12例作为对照组。试验组病例随访9个月后进行GOS评分。结果:32例DAI患者血清NSE水平伤后均有升高,与对照组比较差异有显著性(p<0.05)。预后不良组与恢复良好组之间伤后血清NSE水平比较均有升高,差异有显著性(p<0.05)。结论:血清NSE可以作为早期评估弥漫性轴索损伤患者病情及预后的一个有价值的指标。Objective：To investigate the changes of blood serum NSE levels in patients with diffuse axon injury and its relation with the severity and Clinical outcome of DAI.Methods：32 patients took with DAI were the experimental group,the A group patient 13 examples（GCS grades for 9-12 score） ,the B group patient 19 examples（GCS grades for 3-8 score） ,to determine the NSE blood serum density.the patients which receive a medical examination by the same time outpatient service（passes through the head ct proof not to have brain essence change） .12 examples were take healthy contrast group.All patients of the experimental group had Glasgow outcome scale after 9 months.Result：A significant elevation were observed in serum NSE levels in patient with DAI.and contrast group compares had the remarkable significance（p0.05） ,at the same time the A and B group comparison had the significance difference.There were significant differences of serum NSE levels between B group patients and other groups according to Glasgow outcome scale（P0.05） .the condition more serious,in its serum blood NSE change more obvious,prognosis worse,the patient dying and disability rate was higher.Conclusion：NSE determination regarding the early time appraised the serious degree and the prognosis of DAI have the important clinical reference significance.
