针灸治疗配合康复训练对偏瘫后肩手综合征的疗效观察

摘　　要：目的观察针灸治疗配合康复训练对偏瘫后肩手综合征的治疗效果。方法 82例偏瘫后肩手综合征患者随机分为两组,对照组采用常规药物和康复训练,观察组在对照组基础上加针灸治疗,治疗时间为6周。两组在治疗前后分别评定上肢疼痛、关节活动度、水肿程度和日常生活活动能力等综合评定临床疗效。结果治疗后观察组总有效率为95.1%,对照组总有效率为82.9%,两组比较差异有统计学意义(P<0.05),两组治疗后VAS评分、FMA评分、水肿程度和BI评分较治疗前均有显著改善(P<0.05),观察组疗效明显优于对照组(P<0.05)。结论针灸配合康复训练对偏瘫后肩手综合征具有更好疗效。Objective To investigate the clinical efficacy of acupuncture plus rehabilitation training in treating shoulder - hand syndrome ＇after hemiparalysis. Methods Eighty - two patients with shoulder - hand syndrome after hemiparalysis were randomly allocated to the observation group and the control group. The control group received conventional drug therapy and rehabilitation traips- ing. The nbservation group received acupuncture and rehabilitation training at the same time. The clinical therapeutic effect was evaluated and eompared between the two groups after six weeks according to the degree of pain（ VAS scale） , the range of joint motion（Fugl Meyer scale, FMA） , the degree of edema and the activity in daily life（ Barthel scale, B1）. Results The effectivc rate in the obserration group was 95. 1% , significantly higher than that in the control group,which was 82, 9% （P〈 0.05 ）. There was statistically significant improvement in the degree of pain, the range of joint motion, the degree of edema and the activity in daily life after 6 weks therapy （ P 〈 0.05 ）. The effect was significantly better in the observation group than in the control group （ P 〈 0.05 ）. Conclusions The clinical therapeutic effect of acupuncture plus rehabilitation training for treatment of shoulder - hand syndnnnc after hcmiparalysis is better than that of conventional drug therapy and rehabilitation training in the control group.
