新疆地方性慢性砷中毒119例五年随访结果

摘　　要：本文报告1987年4月对新疆奎屯地方性慢性砷中毒119例五年随访结果。1982年4月曾进行首次调查,1984年10月起当地改良饮用水,其含砷量降至0.045mg/L(原为0.6mg/L),含氟量0.095～1mg/L(原为3.45rag/L)。随访表明,病情好转及痊愈者共88例,占74％,主要表现为砷色素异常的色素沉着消失或减轻,砷角化全部消退者较少。神经系及心血管系症状有所加重。阵发性手足麻木多见,司能与神经炎有关。此外,雷诺氏现象及手足发绀、心电图异常较常见。发现皮肤癌4例,基底细胞上皮瘤3例,表皮内鳞癌l例,迄今尚未见内脏恶性肿瘤病例。本组2例死亡,死因与砷中毒的关系有待进一步研究。本组伴发氟中毒18例,与砷中毒有无协同或拮抗作用尚待观察。Five-year follow-up of 119 cases with endemic chronic arsenicism(ECA) in Xinjiang in April 1987 was reported. Preliminary survey was carried out in April 1982.Drinking water was improved since October 1984 in this area and arsenic level in the water dropped to 0.045 mg/L from original 0.6 mg/L and fluorine to 0.095-1.0mg/L from 3.45mg/L. Results of the follow-up are as following.Among 119 cases with ECA,88 patients released or recovered, accounting for 74 per cent. Major manifestations in those were releasing or disappearing of hyperpigmentation in dyspigmentation. Remission of arsenic keratosis appeared in a few cases, and cardiovascular symtoms were worse than before.Paroxysmal numbness on hands and feet were common and may relate to neuritis, In addition, Raynaud's phenomenon, cyanosis on hands and feet and abnormal ECG were also common.Four cases with skin cancer had been found. Basiloma in three cases, and Bowen's disease in one.No malignant visceral tumor has been found so far. There were two deaths in this survey. Relations between their death and ECA need further study. Eighteen cases accompanied with fluorosis were found in this group. The cooperative and antagonistic effects between them need a further observation.
