血管腔内成形术联合干细胞移植治疗糖尿病足的临床观察

摘　　要：目的比较血管腔内成形术(PTA)联合自体外周血干细胞(APBSC)移植与单纯应用两种方法治疗糖尿病足的效果。方法 2006年8月至2010年12月我院收治住院并明确诊断为糖尿病足的患者45例,随机分为联合组14例、PTA组21例和APBSC组10例,联合组行PTA术后给予患肢外周血干细胞移植,PTA组患者行单纯PTA术,APBSC组患者行APBSC移植治疗。记录各组1、3、6个月的治疗效果。结果联合组6个月内疗效明显优于PTA组和APBSC组(P<0.05);6个月内PTA组复发率呈上升趋势,APBSC组则呈下降趋势;于患肢缺血早期进行干细胞移植疗效明显优于晚期(P<0.05)。结论 PTA与APBSC都是治疗糖尿病足的安全、有效的手段,PTA术后行APBSC移植效果优于单纯应用一种方法。Objective To compare the effect of pereutaneous transluminal angioplasty （PTA） combined autologous peripheral blood stem cells （APBSC） transplantation with that of merely PTA or merely APBSC in treatment of diabetic foot. Methods Nineteen limbs in 14 patients with lower limb ischemia were treated by autologous transplantation of peripheral blood mononuclear cells after the PTA procedure.Twenty-one eases for control group were treated with merely PTA procedure and 10 cases for control group were treated with merely APBSC transplantation. Results Complete response was observed in 16 limbs in combination group（19 limbs）,21 limbs and in PTA group（31 limbs）, 4 limbs in APBSC transplantation group（ 14 limbs）, Ischemia recurred in 2 limbs in combination group, 16 limbs in PTA group, 8 limbs in APBSC transplantation group .There was significant difference of eomplete response and recurrence between combination group and control group（P〈0.05）.Conclusion PTA and APBSC transplantation are the safe and effective methods for diabetic foot. APBSC transplantation after PTA procedure is superior to use a method.
