云南、辽宁省基层医疗卫生机构用药情况的调查分析

摘　　要：目的:了解云南、辽宁省基层医疗卫生机构药品使用情况,探讨基层用药存在的安全性问题,提出相应的政策建议,为干预措施的制定提供依据,以期更好地促进合理用药。方法:采取回顾性方法,对云南、辽宁两省的10个城市社区卫生服务中心和10所乡镇卫生院进行用药调查。随机抽取每个基层卫生机构2007年的处方,共计22356份。将有关数据进行平行双录入,采用SPSS11.5统计软件进行分析,统计方法包括校正t检验、χ2检验、秩和检验等。结果:22356份处方中,大部分处方书写不够规范,仅缺少诊断这一项就超过半数;医疗机构诊治的疾病排前10位的共占所有诊断的92.5%;使用药品品种数为292～1800种,平均594.25种,平均每张处方用药数为3.1种,5种及以上占21.1%,单张处方最多使用的为云南省18种,辽宁省14种;抗生素联用处方比例高达24.9%,最多使用8种;基本药物使用率为75.2%;药品通用名使用率为76.9%。结论:云南、辽宁基层医疗机构存在诸多不合理用药情况,各级政府部门需加以重视和监督管理,有必要采取相应措施加以提高和完善。OBJECTIVE：To investigate the situation of drug use in primary health institution of Yunnan province and Liaoning province,to identify the safety concerns of drug use,to propose appropriate policy and recommendations,and to provide reference for the formulation of interventions measure so as to better promote rational use of drugs.METHODS：With a retrospective method,we investigated the situation of drug use in 10 community health service centers and 10 township hospitals of Yunnan province and Liaoning province,randomly selected the prescriptions from every primary health institution in 2007,a total of 22 356 prescriptions.The relevant data was inputted with parallel double entry,and data statistics was analyzed by SPSS 11.5 software,statistical methods included correction t test,χ2 test,rank-sum test and so on.RESULTS：Prescription writing was not standardized,more than half of prescriptions lacked of diagnosis.Top 10 diseases being diagnosed and treated by medical institutions accounted for 92.5% of all diagnoses.292～1 800 kinds of drugs was used,with average value of 594.25,the average number of drugs per prescription was 3.1,21.1% prescriptions included 5 or more drugs.18 kinds of drugs were used in each prescription at the most in Yunnan province,14 kinds of drugs in Liaoning province.The percentage of prescriptions with two or more antibiotics was 24.9%,up to 8 antibiotics at the most.The percentage of essential drugs was 75.2%,and the percentage of generic name was 76.9%.CONCLUSION：There are many problems of irrational drug use in primary health institution in Yunnan province and Liaoning province.Governments at all levels need to pay more attention and give regulation and supervision.It is necessary to take appropriate measures to improve and perfect this situation.
