经脐单孔腹腔镜胆囊切除术18例体会

摘　　要：目的探讨经脐单孔行腹腔镜胆囊切除术(LC)的安全性及可行性。方法回顾性分析滁州市中西医结合医院2010年3月-2010年7月期间18例经脐单孔腹腔镜胆囊切除术患者的临床资料。结果本组18例手术均获得成功,均未放置引流。手术时间52～135 m in,平均72.5 m in。无漏胆、出血、脐疝、切口感染等并发症。住院2～4 d,切口不拆线,术后随访1～3个月,腹壁疤痕不明显。结论经脐单孔腹腔镜胆囊切除术是可行的,较常规腹腔镜胆囊切除术更显微创优势,腹壁无可见手术疤痕。虽然操作难度较常规腹腔镜胆囊切除术大,但通过熟练的操作可明显缩短手术时间,较传统腹腔镜胆囊切除术更加微创,极具推广价值,有望成为腹腔镜胆囊切除术的新术式。Objective To study the feasibility and safety of transumbilical single-incision laparoscopic cholecystectomy（LC）.Methods The clinical data of 18 patients with transumbilical single-incision laparoscopic cholecystectomy from March 2010 to July 2010 were analyzed retrospectively.Results Eighteen cases were operated successfully with ordinary laparoscopic instruments by transumbilical single-incision laparoscopic cholecystectomy without drainage;the operative time was from 52 to 135 min（average 72.5 min）;No complications,such as biliary leakage,hemorrhage,umbilical hernia and incision infection happened.The hospital delay was 2-4 d.The follow-up of 1 to 3 months showed that the postoperative abdominal wall scar was not obvious.Conclusion Compared with the routine LC,the transumbilical single-incision laparoscopic cholecystectomy has more micro-invasive advantages and not visible abdominal scar.Although the operation is more difficult,the time can be shorted by the skill operation.It is expected to become a new surgical procedure and recommendable.
