心理干预对神经内科住院老年患者心理状态的影响

摘　　要：目的探讨心理干预对神经内科住院老年患者心理状态的影响。方法将神经内科335例60岁以上住院患者随机分为两组,对照组(n=168)常规治疗;研究组(n=167)在常规治疗基础上给予全面全程心理干预;于入院和出院时采用症状自评量表(SCL-90)对两组患者进行心理状态评定分析,并与中国常模比较。结果入院时两组SCL-90各因子评分无差异;出院时,两组评分均有改善(P<0.01),除精神病因子外,研究组SCL-90各因子评分改善值均高于对照组,差异有统计学意义(P<0.01);敌对因子是两组改善比率差异最大的因子;出院时SCL-90各因子分值仍高于中国常模。结论对神经内科住院老年患者,心理干预有助于改善其心理状态,减少患者敌对、焦虑、抑郁情绪,改善医护关系,提高患者的生活质量。Objective To investigate the effect of the psychological intervention on mental condition of elderly patients in inpatient department of neurology.Methods A total of 335 patients over sixty from inpatient department of neurology were randomly divided into two groups： the control group（n=168） and the study group（n=167）.The two groups had the same clinical routine treatments and whole course psychological intervention was added to the study group.The Symptom Check List 90（SCL-90） were used to analyze the mental status before and after the treatment respectively and compared with the noun.Results There was no significant difference of SCL-90 scores between the two groups at the first time.The scores of the study group were significantly lower than those of the control group when re-evaluation was conducted after the treatment（P0.01）.Except psychotic,the comparison showed that the scores of SCL-90 of the two groups both decreased,but those of the study group decreased more significantly（P0.01）.Among them,the hostility was the most significant.But the scores of SCL-90 were higher than the norm（P0.01）.Conclusion Whole course psychological intervention played an important role in the treatment of elderly patients in inpatient department of neurology,and can improve their mental status and quality of life.
