血清胱抑素C对慢性阻塞性肺疾病合并呼吸衰竭行机械通气患者的预后评估

摘　　要：目的探讨血清胱抑素C(CysC)对慢性阻塞性肺疾病(COPD)合并呼吸衰竭行机械通气患者的预后评估价值。方法对该院RICU收治的48例COPD合并呼吸衰竭行机械通气患者的资料进行回顾性分析,根据预后情况将患者分为存活组及死亡组,比较两组血清CysC水平、再根据血清CysC水平将患者分为CysC增高组和CysC正常组,比较两组急性生理学及慢性健康状况(APACHEⅡ)分值、机械通气时间及病死率。结果死亡组血清CysC水平高于存活组(P<0.05);CysC增高组的病死率及APACHEⅡ评分均高于CysC正常组(P<0.05)。结论血清CysC水平能在早期提示COPD合并呼吸衰竭行机械通气患者的预后情况,可考虑指导临床治疗。【Objective】 To assess the value of serum cystatin C（CysC） on prognosis of chronic obstructive pulmonary disease（COPD） patients with respiratory failure and mechanical ventilation.【Methods】 Retrospectively analyzed 48 COPD cases with respiratory failure and mechanical ventilation hospitalized by RICU.According to the prognosis cases were divided into survivors and non-survivors groups,and the level of serum cystatin C was observed.Next,patients were divided into hypo-CysC group and normo-CysC group according to the level of serum CysC.The APACHEⅡ,the time of mechanical ventilation,and mortality rate were observed.【Results】 The level of serum CysC was higher in non-survivors group than that in survivors group（P〈0.05）.Mortality rate and APACHEⅡ score were higher in hypo-CysC group than those in normo-CysC group（both P〈0.05）.【Conclusion】 The level of serum CysC may indicate the prognosis of COPD patients with respiratory failure and mechanical ventilation at the early stage,which should be applied to guiding clinical care.
