牵引推正法治疗神经根型颈椎病30例临床观察

摘　　要：目的:观察牵引推正法对神经根型颈椎病的临床疗效并观察治疗后疗效指数与X线片下颈椎失稳的改善情况的相关性,探索牵引推正法治疗神经根型颈椎病的机制。方法:依据纳入标准收集30例神经根型颈椎病患者,用牵引推正法进行治疗,对每一病例手法治疗10次后依神经根型颈椎病症状和功能评估量表确定疗效指数;同时记录治疗前后颈椎X线片下椎体水平位移值,统计分析牵引推正法治疗神经根型颈椎病的临床疗效,并观察治疗后疗效指数及X线片下颈椎失稳的改善情况的相关性,对两者进行一元线性相关分析。结果:30例患者治疗前后疼痛判断(VAS评分)及主要症状和体征积分比较结果显示牵引推正法是治疗神经根型颈椎病(颈椎失稳)的有效方法,且通过一元线性回归分析,曲线拟合优度较好,R2=0.448,P<0.01,具有统计学意义,疗效指数=0.629+0.184×X线片显示的位移改变。结论:牵引推正法是治疗神经根型颈椎病(颈椎失稳)的有效方法;牵引推正法治疗神经根型颈椎病(颈椎失稳)的疗效与X线片下颈椎失稳的改善情况呈正相关;牵引推正法手法作用机制是通过整复前后移位的颈椎关节,从而达到治疗的目的,并且疗效的优劣与X线片下颈椎前后移位的改善情况呈正相关。Objective： To observate the clinical effect of traction and traumatology therapy in treating cervical spondylotic radiculopathy,and the correlation between the therapeutic index and X-ray improment of cervical vertebral instability.And then explore the mechanism of traction and traumatology therapy in treating cervical spondylotic radiculopathy.Methods： 30 cases of cervical spondylotic radiculopathy were treated with traction andtraumatology for 10 times.The therapeutic indexes were obtained by scoring.And the efficacy of treating cervical vertebral instability was obtained by X-ray.Simple linear regression analysis was adopted to analyze the correlation between them.Results： By simple linear regression analysis,it had come to R^2=0.448,F=22.669,P〈0.01,which stands for statistically significant.Conclusion： The therapy of traction and traumatology is worthy of promoting for its effects on cervical spondylotic radiculopathy.
