脉冲Nd:YAG激光多点布阵术与CO_2激光治疗宫颈糜烂的疗效比较

摘　　要：目的比较脉冲Nd:YAG激光多点布阵术与CO2激光治疗宫颈糜烂的疗效。方法宫颈糜烂患者409例随机分为两组,脉冲Nd:YAG激光多点布阵术治疗208例,CO2激光治疗201例。观察术中创面渗血及不适感。术后随访3个月评价疗效。结果脉冲Nd:YAG激光多点布阵术和CO2激光治疗宫颈糜烂的总有效率分别为98.6%和98.5%,差异无显著性意义(P>0.05);创面渗血脉冲Nd:YAG激光多点布阵术组2例(1.0%),CO2激光组36例(17.9%),差异有显著性意义(P<0.01);治疗过程中出现小腹不适脉冲Nd:YAG激光多点布阵术组33例(15.9%),CO2激光组7例(3.5%),差异有显著性意义(P≤0.01)。结论两种方法治疗宫颈糜烂的效果相近,脉冲Nd:YAG激光多点布阵术的止血作用优于CO2激光,但治疗过程中部分患者有不适感。Objective To study the curative effect and complication of cervical erosion cases with more punctiform distribution treatment method of pulsed Nd：YAG laser or with CO2 laser. Methods In a randomized study,409 patients with cervical erosion were allocated to either pulsed Nd：YAG laser（208 cases） or CO2 laser（201 cases） and to observe their curative effect and complication during operation as ache or discomfort in lower abdomen and slight wound hemorrhage,follow-up for three months after the operation and evaluate curative effect. Results The total curative effective rate of Nd：YAG laser group was 98.6% and that of CO2 laser group was 98.4% respectively.There was no significant difference（P0.05）between the two groups.There were 2 cases with slight wound hemorrhage in pulsed Nd：YAG laser group（0.96%）vs 36 cases in CO2 laser group（17.9%） with a significant difference（P0.001） between the two groups.There were 33 cases（15.9%） with unwell lower abdomen in pulsed Nd：YAG laser group during laser operation,vs 7 cases in CO2 laser group（3.5%） with a significant difference between them（P0.001）. Conclusions The curative effect of two treatment groups with cervical erosion is the same,the effect on hemostasis of pulsed Nd：YAG laser is better than that of CO2 laser,but there are some cases with slight unwell lower abdomen in the pulsed Nd：YAG laser group during laser operation.Whether pulsed Nd：YAG or CO2 laser operation should be performed in cases with cervical erosion much more depends on the patients themselves.
