六味地黄加味方治疗早期糖尿病肾病临床研究

摘　　要：目的观察六味地黄加味方对早期糖尿病肾病(DN)的临床疗效,探讨其作用机制。方法将120例早期DN患者随机分为对照组及治疗组。对照组在常规降糖治疗的同时给予卡托普利25 mg/次,每日3次,视患者血压情况调整用量,合并高血脂加舒降之20 mg/d;治疗组在对照组基础上加服六味地黄加味方治疗。疗程8周,观察患者治疗前后临床症状、24 h蛋白尿、血脂、血液流变学的变化。结果治疗组在降低尿蛋白,改善临床症状、血脂代谢及血液流变学等方面均明显优于对照组(P<0.01或P<0.05)。结论六味地黄加味方可改善早期DN患者的临床症状,减少尿蛋白排泄,降低血液黏稠度,减轻肾损害,从而改善肾功能,其作用机制可能与其改善高凝状态、减少自由基对肾脏结构损伤、改善肾小球基底膜的滤过性有关。Objective To observe the influence of treatment and the mechanism of Modified Liuwei Dihuang on changes in patients with early diabetic nephropathy （EDN）. Methods One hundred and twenty patients with EDN were randomly divided into two groups, the control group and the treatment group. The control group was treated by captopril （25 mg, tid, po, the dose of captopril was changed according to the level of blood pressure） and simvastatin （20 mg, qd, po） if having hyperlipemia in addition to the conventional treatments of controlling blood sugar. The treatment group was treated with the Modified Liuwei Dihuang based on the therapy of the control group for 8 weeks. The changes of clinical symptoms, 24 h proteinuria, blood lipid and blood viscosity before and after treatment were observed. Results The treatment group was superior to the control group on reducing proteinuria, clinical symptoms, blood lipid and blood viscosity with significant difference （P 〈0.05 or P 〈0.01）. Conclusion The Modified Liuwei Dihuang decoction can improve clinical symptoms, decrease UAER, blood viscosity and blood lipid, improve microcirculation, and ease renal injury, thus improve renal function of EDN patients. The effective mechanism may be related to improving the hypercoagulable state of blood, helping protect kidney from harmful free radicals and improving the filtration of glomerular basement membrane.
