不同形态支撑体植入对再造阴茎龟头形态的影响

摘　　要：目的比较3种不同形状支撑体对再造阴茎龟头形态的影响,以期找到能长期维持再造阴茎丰满龟头与良好外形的合适支撑体。方法将直条状自体肋软骨、T形自体肋软骨和自体直条肋软骨远端相拼膨体聚四氟乙烯(混合型)等3种不同形状支撑体,一期植入再造阴茎,对患者进行长期随访,观察不同支撑体对再造阴茎龟头形态变化的影响。结果采用直条状或T型肋软骨作为支撑体植入者,再造阴茎远期变化显著,龟头缩小,组织松软,尿道口后退,部分呈鲨鱼头状或鸭嘴状畸形。混合型支撑体植入者中,再造阴茎龟头远期形态变化较小,外形丰满,尿道外口后退不明显。结论阴茎再造术中植入混合型支撑体,再造阴茎能维持其龟头形态丰满,远期形态变化不显著,应在阴茎再造术中推广应用。Objective To compare the influence of 3 different shapes of implanted stiffener on the appearance of the constructed neo-glans and to find out a suitable stiffener for phalloplasty. Methods Three different shapes of stiffener （strip shaped autologous rib cartilage, ＂T＂ shaped autologous rib cartilage and strip shaped autologous rib cartilage combining ＂C＂ shaped e-PTFE） were implanted into the constructed phallus respectively in 59 patients. During the follow up period （1 year to 20 years）, the eonfigurational changes of the neo-glans were observed. Results In patients that strip or ＂T＂ shaped autologous rib cartilage were used as the stiffener, the original appearance of the constructed neo-glans could not be maintained in the long term and the urethral meatus retracted without urethral stricture. In patients that mixed stiffener was used the constructed glans changed mildly in the long term and no urethral meatus retraction occurred. Conclusion Using the mixed e-PTFE and autologous rib cartilage as the implanted stiffener for phalloplasty can maintain the original good appearance of the constructed neoglans in a long term and should be advocated to be used in phalloplasty.
