静脉注射胺碘酮转复急性冠脉综合征并发快速房颤的临床研究

摘　　要：目的观察静脉应用胺碘酮对急性冠脉综合征（acute coronary syndrome，ACS）并发快速心房颤动（atrialfibrillation，AF）总者的临床价值。方法32例ACS并快速房颤患者．先给静脉推注负荷量胺碘酮150mg，继之持续静脉泵入，观察房颤转复、心室率、血压改变及不良反应。结果其中24例患者在24h内转复为窭性心律。转复率为75％。32例患者用药后30min、6h、24h心室率分别为（118±11）；次／min、（84±10）；次／min、（73±9）次／min，较用药前（128±13）次／min显著降低。血压无明显变化。1例出现窦缓，1例出现窦性停搏，1例出现I度房室传导阻滞，停药后均自行恢复．结论静脉应用胺碘酮治疗急性冠脉综合征并发快速房颤．是一种安全、有效的临床措施。Objective To determine the value of amiodarone in patients of acute coronary syndrome （ACS） with fast atrial fibrillation （Ai）. Methods 150mg amiodarone was intravenously load in 32 ACS patients with new fast atrial fibrillation, then continuously pumped amiodarone. The efficacy of amiodarone to reversion the atrial fibrillation, control the ventricular rate, change blood pressure and the side effeest of amiodarene were observed. Results 24 case （75% patients） were reversed to sinus rhythm. The ventrieular rate at 30rain, 6 hour, 24 hour was respectively （118±1）beats/min, （84±10）beats/min, （73±9）beats/rain, which were significantly lower than the heart rate of （128±13）beats/min when the amiodarone was administered. Blood pressure did not show significant difference （P〈0.05）. Sinus bradycardia, Sinus bradyeardia and [ o AV block was respectively observed in one patient and recover normal when amiodarone was stopped. Conclusion Intravenous amiodarone is a safe and effective clinie measure to fast atrial fibrillation patients with ACS.
