开环式痔上黏膜吻合术与Milligan-Morgan术治疗混合痔的临床疗效评价

摘　　要：目的评价开环式痔上黏膜吻合术(TST)和Milligan-Morgan(M-M)术治疗混合痔的临床疗效。方法将60例混合痔患者随机分为TST组和M-M组,每组各30例;TST组采用TST术治疗,M-M组采用M-M术治疗,比较两组临床疗效。结果 TST组、M-M组总有效率均为100%;组间临床疗效比较,差异无统计学意义(P>0.05)。两组手术时间、术中出血情况、住院时间和住院费用比较,差异均有统计学意义(P<0.05)。两组术后出血及3个月后随访脱垂等并发症积分比较,差异无统计学意义(P>0.05);两组术后水肿、疼痛、3个月后随访皮赘残留等并发症积分比较,差异有统计学意义(P<0.05)。结论 TST术和M-M术均能有效治疗混合痔,且TST术治疗混合痔具有操作简单、疗效确切、并发症少和痛苦小的特点,具有较好的临床实用价值。Objective To compare the clinical efficacy of tissue selecting therapy （TST） and Milligan-Morgan （ M-M ） in the treatment of mixed hemorrhoids. Methods Sixty cases of mixed hemorrhoids were randomly divided into treatment group and control group, 30 cases in each group ; the treatment group was treated with the tissue selecting therapy, and the control group was treated with the Milligan-Morgan. The curative effects in the two groups was compared. Results The total effective rates were 100% in both groups, with no significant difference between them （ P 〉 0.05 ）. There were significant differences in operative time, hemorrhage during operation, hospitalization period and cost between the two groups（ P 〈 0.05 ）, no difference in the symptom scores of post-operative hemorrhage and rectal prolapse during three-month follow-up（ P 〉 0.05 ）, and significant differences in the symptom scores of post-operative edema pain and skin tag left （ P 〈 0.05 ）. Conclusion Both TST and M-M have good effects on mixed hemorrhoids; but tissue selecting therapy presents with simple operation, fewer complications and less suffering.
