血管内栓塞治疗颅内前交通微小动脉瘤20例

摘　　要：【目的】探讨前交通微小动脉瘤介入治疗的疗效、安全性及技术要点。【方法】以可解脱弹簧圈栓塞治疗20例患者（20枚）前交通微小动脉瘤。【结果】栓塞治疗后即刻脑血管造影，动脉瘤致密填塞13例，动脉瘤部分栓塞7例。1例Hunt—Hess分级为V级患者死亡，余患者出院前GOS（GlasgowOutcomeScale）评分：5分14例，4分2例，3分2例，1分1例。【结论】微弹簧圈血管内栓塞是前交通微小动脉瘤较好的治疗手段。治疗关键是合理选择介入治疗方案。[Objective] To explore the efficacy, safety and technical key point of interventional therapy of anterior communicating micro-aneurysms. [Methods]Twenty patients with 20 micro-aneurysms of anterior communicating arteries were treated by endovascular embolization with detachable spring coils. [Results]The instant angiogvaphy after embolization showed that 13 aneurysms were fully embolized and 7 aneurysms were partly embolized. One patient with grade V （Hunt-Hess Scale） died. According to Glasgow outcome scale, 14 patients got 5, and 2 patients got 2, and 2 patients got 3, and one patient got 1 before discharged from the hospital. [Conclusion] Endovascular embolization with detachable spring coils is a better method for the treatment of anterior communicating micro-aneurysms. Reasonably choosing the interventional therapy schedule is the key of treatment.
