摘 要:目的:探讨冻融胚胎移植周期决定移植日与移植当日,子宫内膜的厚度变化及形态转化与妊娠结局的关系,评价阴道超声下子宫内膜形态变化与否能否用于预测临床妊娠。方法:回顾性分析2009年10~12月生殖医学中心冻融胚胎移植共482周期,除常规分析外,记录并比较决定移植日及移植当日内膜厚度、形态。结果:482个周期中比较决定移植日及移植当日子宫内膜,人工周期组内膜转化率高于其他两组,形态转化充分组妊娠率较高(P<0.05);移植当日为C型内膜者妊娠率稍高,但差异无统计学意义。结论:内膜转化充分组临床妊娠率高于转化不明显组,提示充分转化内膜更有利于胚胎着床,移植日内膜情况更接近胚胎着床时状态,并认为增加移植日的超声监测有一定临床意义。Objective: To explore the relationship between the change of endometrial thickness, pattern transformation and pregnancy outcome on the determination day of transplantation and transplantation day during frozen - thawed embryos transfer cycles, evaluate the morphological change of endometrium under transvaginal ultrasound and explore whether it can be used to predict clinical pregnancy. Methods: 482 frozen - thawed embryos transfer cycles in center of reproductive medicine from October to December in 2009 were analyzed retrospectively, the thickness and morphology of endometrium on the determination day of transplantation and transplantation day were recorded and compared besides routine analysis. Results: The transformation rates of endometrium on the determination day of transplantation and transplantation day in artificial cycle group were significantly higher than those in the other groups, the pregnancy rate in fully transformed endometrium group were higher ( P 〈 0. 05 ) ; the pregnancy rate in cases with type C endometrium on transplantation day was higher , but there was no significant difference. Conclusion: The clinical pregnancy rate in fully transformed endometrium group is higher than that in inconspicuous transformed endometrium group, which indicates that fully transformed endometrium is helpful to embryo implantation, the endometrial situation on transplantation day is close to the situation of embryo implantation, it is of clinical significance to enhance uhrasonie monitoring on transplantation day.
