单孔双管闭式引流治疗慢性硬膜下血肿

摘　　要：目的探讨单孔双管闭式引流治疗慢性硬膜下血肿(CSDH)的临床治疗效果。方法慢性硬膜下血肿患者随机按单孔单管额部引流组,单孔单管顶枕部引流组和单孔双管引流组,各选取30例。分析其临床资料和影像学资料。结果单孔双管引流组中线恢复好于其它二组(P<0.01),颅内积气和残留液体量三组比较均有显著差异(P<0.01)。结论单孔双管闭式引流治疗慢性硬膜下血肿具有操作简单、创伤小,引流完全,可减少术后颅内积气和残留液体量,促进受压脑组织膨起。术后应尽早拔除引流管。Objective To explore the chronic subdural hematom（CSDH） by burr operative experience and curative effect of treatment of hole drainage with two tube. Methods A retrospective study was performed on 90 patients with CSDH underwent burr hole closed-drainage at our institution in the last 4 years, with comparing the clinical data and imaging data of three different primary surgical methods ： closed-drainage of hematoma through one tube in forehead （ n = 30） ; closed-drainage of hematoma through one tube in parietooccipial （ n = 30） ; the last closed-drainage of hem-atoma through double tube in forehead and parietooccipial （ n = 30 ）. Results All groups were not significantly different in sex, age and preoperative hematoma volume. In the closed drainage by doubletube group postoperative midline recovery,pneumocephalus and the residual liquid volume significantly decreased compared to the other groups. Conclusion Burrhole closed-drainage with double tube is a technically easy ,highly effective,safe and cost-efficient treatment for CSDH.
