摘 要:目的探讨犬行体外腹主动脉末端阻断对循环系统的影响,为骨盆及以下部位动脉性大出血在急救时临时止血提供实验依据。方法取杂种犬5只,前肢动、静脉插管,后肢动、静脉穿刺置管,分别连接多功能生理监护仪。应用自制体外腹主动脉阻断装置,自体外将腹主动脉远段向脊椎椎体挤压至血流完全阻断,观测犬心率(HR)、中心静脉压(CVP)、前肢动脉收缩压(SBP)、舒张压(DBP)、平均动脉压(MAP);后肢SBP、DBP、髂静脉压等指标变化。结果阻断动脉开放后,前肢SBP、DBP、MAP逐渐降至阻断前水平并趋于平稳,CVP先略有降低,30 min后恢复至阻断前水平并趋于平稳。后肢股动脉压在腹主动阻断后持续维持在33~42 mmHg之间;髂静脉压先稍升高,5 min后略降低,动脉开放后逐渐恢复至阻断前水平并趋于平稳。结论犬体外腹主动脉末端阻断30 min,阻断前后动物上半体循环基本稳定;动脉开放后,下半体循环恢复。Objective To investigate the impact of extracorporeal blockage of abdominal aortic end on canine circulation system,and to provide experimental evidence for temporarily emergent control of fracture-induced arterial hemorrhoea at or below pelvis.Methods Five hybrid dogs were anesthetized before the tracheal intubation.A self-made extracorporeal appliance was used to press the distal abdominal aorta towards canine vertebra in vitro until the blood flow was completely blocked.The changes of DBP,MAP and SBP of the forelimbs,HR and CVP were observed.The pressure of iliac vein,SBP and DBP of the posterior limbs were also recorded.Results When the blood flow was restored,SBP,DBP and MAP of the forelimbs returned to the level of pre-blockage and tended to be stable gradually.The CVP initially showed a slight decrease,but returned to the level of pre-blockage 30 min later and had a tendency of stability.However,the arterial pressure of the posterior limbs was persistently maintained between 33-42 mmHg after the blockage.The pressure of iliac vein showed a little increase initially.But 5 min later,it decreased slightly.When the blockage was removed,it gradually returned to the same level of pre-blockage and tended to be stable.Conclusion The circulation of the canine upper body basically remains stable before and after the extracorporeal blockage of abdominal aortic end for 30 min.When the blockage is taken away,the circulation of the lower body can be restored.
