摘　　要：目的探讨显微内窥镜下椎间盘切除术(MED)治疗腰椎间盘突出症的技术改进及远期临床效果。方法回顾性分析83例(101个椎间盘)腰椎间盘突出症经MED治疗的效果。结果 83例均完成镜下手术操作,无神经根或脊髓硬膜损伤并发症,随访0.5～7年,采用M annab评定标准进行分级,结果优74例,良6例,可1例,差2例。结论 MED技术是一种安全、有效的治疗腰椎间盘突出症的手术方法,创伤小,出血少,术后恢复快,远期疗效满意。Objective To explore the technical improvement of microendoscopic discectomy（MED）for the treatment of intervertebral disc herniation,and to evaluate the long-term clinical effect.Methods A retrospective analysis was made in 83 cases（101 lumbar discs）treated by MED.Results MED was completed in all the 83 cases without complications,such as injuries of nervous roots and spinal dura.The patients were followed up for 6 months to 7 years.Based on Mannab criteria,excellent effect was achieved in 74 cases,good effect in 6 cases,fair effect in 1 case and poor effect in 2 cases.Conclusion MED is a safe procedure for lumbar intervertebral disc herniation with minimal invasion,low bleeding,fast postoperative recovery and long-term satisfactory curative effect.
