摘  要:目的观察手足口病患儿心脏收缩时间间期(STI)的变化,探讨其与临床表现的关系。方法利用二维多普勒超声对16例手足口病(手足口病组)、14例手足口病合并脑干脑炎(脑干脑炎组)、12例手足口病合并肺出血(肺出血组)及14例正常儿(正常组)进行二维超声及STI测定,分析STI改变与临床表现的关系。结果正常组、手足口病组、脑干脑炎组及肺出血组二维超声心动图检查指标比较,差异均无统计学意义(P>0.05)。脑干脑炎组右心室射血前期时间(RPEP)较正常组及手足口病组均明显延长,差异有统计学意义(P<0.05);肺出血组较正常组、手足口病组及脑干脑炎组RPEP明显延长,右心室射血时间(RVET)明显缩短,RPEP/RVET明显增大,左心室射血前期时间(LPEP)明显延长,差异均有统计学意义(P<0.05)。各组间RPEP/RVET增大的人数比较差异有统计学意义(χ2=23.9630,P<0.001),Pearson相关系数P=0.5937。结论重症手足口病患儿右心室STI改变明显,RPEP/RVET与病情有一定的相关性,可以作为预测手足口病合并肺出血的参考指标。Objective To observe the alterations of systolic time interval (STI) in patients with hand - foot - mouth disease (HFMD) and to explore their relationship with clinical manifestations. Methods Tow - dimensional and Doppler echo- cardiography were employed to detect the left and right ventricular STI in 16 cases of hand - foot - mouth disease, 14 cases of HFMD complicated with brainstem encephalitis, 12 cases of pulmonary hemorrhage as well as 14 normal controls. The relationship between STI alterations and clinical manifestations of the patients was analyzed. Results In the acute stage, two - dimensional echocardiography indices among the normal group, the HFMD group, the brainstem encephalitis group and the pulmonary hemor- rhage group showed no statistical differences (P 〉 0. 05) . In comparison with the HFMD group and the normal group, patients in the brainstem encephalitis group displayed longer right ventricular pre - ejection period (RPEP) ( P 〈0. 05 ) . The pulmonary hemorrhage group showed longer RPEP, shorter right ventricular ejection time ( RVET), increased RPEP/RVET ratio, and lon- ger left ventricular pre- ejection period (LPEP) in comparison to the HFMD group, the brainstem encephalitis group and the normal group (P 〈 0. 05) . Numbers of patients with increased RPEP/RVET ratios were significantly different among groups (X2 = 23. 9630, P 〈 0. 001 ) . Pearson correlation coefficient was P = O. 5937. Conclusion Obvious STI alterations exist in the right ventricular of severe HFMD patients. Abnormal RPEP/RVET ratio is closely related to clinical manifestations and might be used as an indicator for predicting pulmonary hemorrhage in HFMD patients.
