直立不耐受患儿生活质量调查分析

摘　　要：目的了解直立不耐受患儿的生活质量现状,探讨直立不耐受对患儿生活质量的影响。方法选择北京大学第一医院儿科就诊的直立不耐受患儿74例为病例组,从北京中小学生体检正常者中,按与病例组1∶1匹配选择对照组。匹配因素是性别、居住地、是否独生子女、学习成绩及性格。采用一般情况问卷、直立不耐受症状严重程度评分表、特质应对方式问卷及儿少主观生活质量问卷对所有儿童进行调查。结果患儿积极应对和消极应对得分与对照组儿童比较,差异均无统计学意义(P>0.05)。两组儿童生活质量总体满意度、认知满意度、情感满意度比较,差异均有统计学意义(P<0.05)。患儿生活质量总体满意度得分低于对照组儿童,差异有统计学意义(P<0.05)。结论直立不耐受对患儿生活质量造成不良影响,使其生活质量下降。因此,临床工作者不仅应关注患儿疾病的本身,更应该关注患儿主观上的不适症状,有针对性地提供多方面的照顾和护理,以促进患儿的身心健康发展。Objective To understand the current situation concerning life quality of children with orthostatic intoler- ance and to discuss the influence of orthostatic intolerance on the life quality of these children. Methods Seventy - four＇ortho- static intolerance patients from our department were enrolled as the case group, with the same number of normal pupil physical ex- amination subjects matched for sex, inhabitats, siblings, school performance, and character enrolled as the control group. All children were surveyed with general situation questionnaire, orthostatic intolerance symptom severity score, trait coping style questionnaire, and self - report life quality questionnaire. Results Scores of actively and passively coping showed no statistical differences between the ease group and the control group （ P 〉 0. 05 ）. Scores of satisfaction in general, cognition satisfaction and emotion satisfaction were significantly different between the two groups （ P 〈 0.05 ）. Scores for satisfaction of life quality in gen- eral were lower in the case group than in the control group （ P 〈 0. 05 ）. Conclusion Orthostatic intolerance negatively influ- ences the life quality of the patients and decreases their life quality. Therefore, medical practitioners should pay attention to not only the disease itself but also the self - reported uncomfortable symptoms. Multifaeeted care should be delivered so as to promote the physical and psychological health of the patients.
