“社区-急救-医院”院前急救模式的应用及效果评价

摘　　要：目的建立新型的"社区-急救-医院"三级急救模式,探索并评价在新型急救模式下社区高血压病患者的急救效果。方法将北京市某城区10家社区卫生服务中心分为试验组和对照组,采用分层随机抽样的方法选取其中的3 665名高血压病患者作为研究对象,对试验组社区进行为期1年的试验性干预,对照组按原有方式进行常规的社区干预,于干预前后进行问卷调查,对比两种模式下的救治效果。结果通过社区干预,试验组高血压病患者的血压控制率、高血压并发症/急症的知晓率以及"社区健康通"的使用率较对照组明显上升,而急症发生率、就诊率的增长幅度较对照组有所缓解。结论在"社区-急救-医院"三级急救模式下,社区居民能够增强对高血压急症的知晓,降低急症的发生率;社区卫生服务人员能够参与并解决部分急症病例,使有限的院前急救资源得到合理应用,因此应在社区中大力推广。Objective To establish a new three-level emergency mode of ＂community-emergency-general hospital＂,and to explore and evaluate its emergent treatment effect in hypertensive patients.Methods Ten community health service centers randomly selected from an urban area in Beijing were divided as trial and control groups.The stratified random sampling method was applied to select 3 665 hypertensive patients as subjects.The patients in the trial group were given community pilot intervention for one year,and those in the control group were given community conventional intervention.A questionnaire survey was conducted before and after the intervention to compare the treatment effects of two modes.Results Through community intervention,for patients with hypertension in the trial group blood pressure control rate,their awareness levels on complications and emergency symptoms of hypertension,as well as the usage rate of ＂Community health service system＂ were significantly increased,and the rate of emergency incidence and visiting rate growth decreased as compared with the control group.Conclusion Under the three-level emergency mode of ＂community-emergency-general hospital＂,the community residents can increase the awareness on emergency of hypertension,and reduce the rate of emergency incidence.The community health providers can participate in and resolve some urgent cases,and thus the limited resource of pre-hospital first-aid can be reasonably utilized.Therefore,the ＂community-emergency-general hospital＂ mode should be greatly popularized in community.
