摘  要:目的探讨高血压病患者动态血压节律异常对脉搏波传导速度(PWV)、利钠肽及左室质量的影响。方法根据24 h动态血压结果将100例高血压病患者分为杓型高血压组与非杓型高血压组,分别进行了PWV、血心房利钠肽(ANP)、脑钠肽(BNP)及超声心动图测定。结果杓型高血压组与非杓型高血压组的PWV、ANP、BNP及左室质量指数(LVMI)均显著高于对照组,差异有统计学意义(P<0.01);其中非杓型高血压组显著高于杓型高血压组,差异有统计学意义(P<0.01)。ANP、BNP浓度与LVMI呈正相关(r值分别为0.46、0.68,P<0.01)。结论高血压患者血压昼夜节律异常者往往伴有外周血管及心脏靶器官损害,PWV值及利钠肽水平可反映高血压病患者血压节律的异常。Objective To evaluate the effects of abnormal circadian rhythm on pulse wave velocity(PWV),natriuretic peptide and left ventricular mass of patients with essential hypertension.Methods According to ambulatory blood pressure in 24-hours,100 with essential hypertension(EH) were divided into two groups: dipper and non-dipper.The PWV,atrial natriuretic peptide(ANP),brain natriutetic peptide(BNP) and left ventricular mass index(LVMI) were measured.Results The PWV,ANP,BNP levels and LVMI in EH patients were significantly higher than those in normal controls(P〈0.01),and the PWV,ANP,BNP levels and LVMI in the dipper group were significantly higher than those in the non-dipper group(P〈0.01).ANP and BNP were positively correlated with LVMI in EH patients(r=0.46,0.68,P〈0.01).Conclusion Hypertensive patients with non-dipper pattern often brings on damage to the peripheral blood vessels and target organs.The increased levels of PWV and natriuretic peptide could reflect the abnormality of blood pressure rhythm in EH patients.
