摘  要:目的评价呼吸机环路给氧在急性呼吸衰竭机械通气患者中的疗效及安全性。方法 77例急性呼吸衰竭患者应用随机数字表法分为A、B两组,两组均在其他治疗基础上予以气管插管接呼吸机通气。A组39例采用呼吸机环路给氧,B组38例采用常规氧源供氧,比较两组患者呼吸衰竭改善情况、呼吸机相关肺损伤(VALI)发生情况。结果治疗后两组患者全身症状及动脉血气均明显改善,血气主要指标pH值、动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)与治疗前相比差异均有统计学意义(P<0.05);两组间比较差异无统计学意义(P>0.05),两组疗效一致,均未出现呼吸机相关肺损伤。结论呼吸机环路给氧疗效肯定,出现呼吸机相关肺损伤等并发症的可能性小,可作为紧急情况下的权宜措施。Objective To assess the clinical effects and safety of oxygen administration via ventilator loop in mechanical ventilation of patients with acute respiratory failure.Methods Seventy-seven patients with acute respiratory failure were randomly divided into two groups(group A,n=39;group B,n=38),both underwent tracheal intubation and mechanical ventilation in addition to sustained treatment.Oxygen was administrated via ventilator loop in group A and was administrated using the routine method in group B.Therapeutic effects and ventilator-associated lung injury(VALI) were compared between the two groups.Results Systimatic symptoms and results from blood gas analysis were improved in both groups.Main indices of blood gas analysis including pH,PaO2,and PaCO2 were significantly different from those before treatment(P〈0.05) and were not statistically different between the two groups(P〉0.05).VALI was absent in either group.Conclusion Oxygen administration via ventilator loop during the mechanical ventilation of patients with respiratory failure is effective and safe.It can be used as an alternative method under emergent circumstances.
