摘  要:目的评价肝素在烧伤治疗中的作用。方法以英文关键词"heparin,burns,scald,thermalinjury"计算机检索MEDLINE、EMbase、CINAHL、Cochrane图书馆、WebofScience和BIOSIS,以"烧伤、烫伤、热力伤、肝素"为中文关键词计算机检索中国期刊全文数据库、中国学术期刊网络出版总库、中国生物医学文献数据库(网络版)和中国优秀博硕士学位论文全文数据库中关于肝素在烧伤治疗中应用的对照试验研究,同时筛检纳入文献的参考文献。严格评价后对纳入的研究进行Meta分析。结果共纳入17个研究,文献质量评价较低。肝素被用于火焰烧伤、热力烧伤、化学性烧伤、电烧伤及吸入性损伤创面,采取局部和全身应用方式。对应用肝素后烧伤患者死亡率、创面感染、瘢痕增生、愈合时间做Meta分析均有统计学意义。8篇研究报道了出血等不良反应,但Meta分析表明差异无统计学意义(P>0.05)。结论已有的研究表明肝素在烧伤治疗中具有降低死亡率,控制局部创面感染,促进创面愈合,缩短住院时间,预防瘢痕增生等作用。Objective To evaluate the effect of heparin on treating burn injury. Methods The MEDLINE, EMbase, CINAHL, Coehrane Database, Web of Science and BIOSIS were searched to collect controlled trials. Principal keywords included heparin, bums, scald and thermal injury. Additional searches involved the reference lists of included studies. The quality of the studies was critically appraised and six Meta-analyses were conducted. Results Seventeen studies were indicated, most of which with poor study quality. Heparin was used to treat the wound caused by flame bums, thermal bums, chemical bums, elec- tric burns and inhalation bums. Four Meta-analyses were performed about the effect of heparin on mortality, scar prevention, infection and wound healing and showed statistical significance. Eight studies revealed the side effect such as bleeding, but had no significant change ( P 〉 0.05 ). Conclusion According to the existing researches, heparin had a beneficial impact on mortality, scar prevention, infection and wound healing.
