摘 要:目的了解中山地区铜绿假单胞菌β-内酰胺酶类药物的耐药表型和基因分布及其基因型与表型的相关性。方法采用VITEK-2Compact高级专家系统判定2010年1-9月分离的896株铜绿假单胞菌对β-内酰胺酶类药物的耐药表型,用PCR方法检测25株铜绿假单胞菌的13种β-内酰胺酶基因。结果 896株铜绿假单胞菌对β-内酰胺酶类药物的耐药表型共有9种,主要表型为高水平耐药+碳青霉烯酶(不渗透性)占47.3%;连续分离的25株铜绿假单胞菌中耐药表型共有4种,主要表型为高水平耐药+碳青霉烯酶12株占48.0%;检出阳性耐药基因9种:blaOXA-10、blaDHA基因阳性率均为4.0%,blaSHV20.0%,blaCARB12.0%,blaGIM 8.0%,blaGES8.0%,blaTEM16.0%,oprD2基因缺失率60.0%,未检出blaIMP,blaVIM,blaPER,blaVEB,blaSPM。结论临床分离的铜绿假单胞菌β-内酰胺酶类药物耐药表型和基因型基本相符,推测该地区β-内酰胺酶类药物耐药的主要原因是β-内酰胺酶基因的存在导致。OBJECTIVE To investigate the distribution and correlation of β-lactmases drug resistant phenotypes and genotypes of Pseudomonas aeruginosa(PAE) in the zhongshan district.METHODS The senior expert system of VITEK 2 Compact was used to judge the β-lactamase drug resistant phenotypes in 896 strains of PAE from Jan 2010 to Sep 2010,and 13 kinds of β-lactamase genes were detected by PCR in the 25 strains of separated from July to Sep 2010.RESULTS There were 9 species ofβ-lactamase drug-resistant phenotypes modes in 896 strains PAE:the main was high level resistant+carbapenems(impermeability)(47.3%);There were 4 species of β-lactamase drug-resistant phenotypes modes in 25 strains PAE separated continuously:high level resistant+carbapenems(impermeability) accounted for 48%;and 9 kinds of drug resistant genes were positive:blaOXA-10 and blaDHA was 4%,blaSHV 20.0%,a blaCARB 12.0%,blaGIM 8.0%,blaGES 8.0%,blaTEM 16.0%(4/25),and the rate of oprD2 gene deletion was 60%.blaIMP,blaVIM,blaPER,blaVEB and blaSPM were negative.CONCLUSION The β-lactamase drug resistant phenotypes and genotypes of PAE separated from clinical are basically consistent,which indicate the main reason causes the aminoglycoside drug resistance is the β-lactamase gene.
