原发性高血压患者夜间血压水平与左室重塑及高敏C-反应蛋白的关系

摘　　要：目的探讨原发性高血压患者(EH)夜间血压水平与左室重塑及高敏C-反应蛋白(hs-CRP)的关系。方法 140例EH按夜间平均血压增高与不增高分为两组。观察组62例,对照组78例,进行24h动态血压监测、超声心动图、hs-CRP测定。结果观察组夜间的最高收缩压(SBP)及舒张压(DBP)、最低SBP、平均SBP和DBP与对照组比较,差异有统计学意义(P<0.05);观察组左心房内径(LAD)、室间隔厚度(IVST)、左心室后壁厚度(LVPWT)、左心室重量(LVM)、左心室重量指数(LVMI),与对照组比较有统计学意义(P<0.05))。观察组hs-CRP和对照组比较明显升高(P<0.05)。结论 EH夜间平均血压增高患者,左室重塑较重,hs-CRP水平增高,炎症介质可能加重了左室重塑的过程。Objective To investigate the relationship between the nighttime blood pressure level,left ventricular remodeling,and high-sensitivity C-reactive protein（hs-CRP） in patients（pts） with essential hypertension（EH）.Methods The 24 hours ambulatory blood pressure monitoring was performed in 140 pts with EH.Based on blood pressure increase at nighttime,the pts were divided into observed group（n=62） and control group（n=78）.The 24 hours ambulatory blood pressure monitoring,heart echocardiography,electrocardiogram,and hs-CRP were studied.Results The highest level of systolic blood pressure（SBP） and diastolic blood pressure（DBP）,the lowest level of SBP,the average level of SBP and DBP at nighttime in observed group were higher than that in control group（P0.05）.The left atrial diameter,ventricular septal thickness,left ventricular posterior wall,left ventricular mass,and left ventricular mass index in observed group were higher than that in control group（P0.05）.The level of hs-CRP in observed group was obviously higher than that in control group（P0.05）.Conclusion For EH pts with nighttime average blood pressure increase,the left ventricular heart remodeling was seriously aggravated,and hs-CRP levels were increased.Inflammation medium might play an important role in the remodeling of left ventricle.
