福特蒙、果纳芬和丽申宝在体外受精-胚胎移植中超促排卵效果分析

摘　　要：目的探讨福特蒙、果纳芬、丽申宝在体外受精-胚胎移植(IVF/ICSI-ET)中超促排卵的效果。方法回顾分析326个IVF/ICSI-ET周期的病例资料,分为福特蒙组58例、果纳芬组83例、丽申宝组185例。卵泡监测、取卵I、VF/ICSI-ET等均按本中心常规进行,观察三组获卵数、卵子成熟率、受精率、优质胚胎率、冷冻胚胎率及妊娠率。结果①三组患者年龄和不孕原因比较差异无统计学意义(P>0.05),其中福特蒙组不孕年限时间最短(4.05±3.76,P<0.05),三组获卵数、妊娠率差异无统计学意义。②三组患者按年龄分层,<35岁中福特蒙组冷冻胚胎率最高(P<0.05),年龄≥35岁中福特蒙组优质胚胎率最高(P<0.05)。③三组患者按长(短)方案分层,长方案中福特蒙组冷冻胚胎率最高(P<0.05),短方案中三组各方面比较差异无统计学意义(P>0.05)。④使用果纳芬的患者卵子成熟率和受精率最高(P<0.05)。结论在治疗35岁以上IVF患者时,福特蒙可能更有优势,丽申宝更具有价格优势,可根据患者的具体情况制定个体化促排卵方案。Objective To compare the effect of highly purified urinary FSH recombinant FSH,and urinary FSH in IVF-ICSI/ET undergoing the controlled ovarian hyperstimulation.Methods Retrospective analysis of 326 medical records of IVF-ET/ICSI cycle which were divided into these groups： highly purified urinary FSH group（n=58） recombinant FSH group（n=83）,and urinary FSH group（n=185）.The oocytes,oocyte maturation rate,fertilization rate,good embryo rate,frozen embryos rate and pregnancy rate were observed in three groups.Results The patients in three groups in age and causes of infertility were no significant difference（P0.05）,and highly purified urinary FSH group which had the shortest period of infertility（4.05±3.76）（P0.05）.There was no significant difference in pregnancy rate and retrieved oocytes in three groups.The patients in three groups stratified by age： Less than 35-year-old,frozen embryos rate of highly purified urinary FSH group was higher than that in other groups（P0.05）;Older than 35 years,the high quality embryos rate of highly purified urinary FSH group was higher than that in other groups（P0.05）.The patients in three groups stratified according to long / short protocol： in long protocol,the days of Gn（P0.05） and frozen embryos rate（P0.05） were the highest in highly purified urinary FSH group;in short protocol,all aspects of the three groups showed no significant difference（P0.05）.Oocyte maturation rate and fertilization rate（P0.05） were the highest in all patients with recombinant FSH.Conclusion Highly purified urinary FSH may be more advantages used by patients over 35 years of age in the treatment of IVF.Recombinant FSH has high purity,it may be more advantages used by patients less than 35-year-old.
