重症手足口病机械通气患儿并发应激性胃溃疡的急救与护理

摘　　要：目的总结28例重症手足口病机械通气患儿并发应激性溃疡的急救与护理。方法62例重症手足口机械通气患儿均接受鼻饲营养支持，28例胃管内抽出咖啡色液体并发应激性溃疡。通过静滴奥美拉唑、凝血酶鼻饲、思密达鼻饲、冷盐水洗胃及盐水配制奶粉鼻饲方法进行急救与护理。结果28例患儿无1例有便血、上消化道大出血情况，1～3d溃疡治愈消失。结论重症手足口病机械通气患儿并发应激性溃疡可通过静滴奥美拉唑、药物及奶粉鼻饲、冷盐水洗胃及盐水配制奶粉鼻饲方法治愈。Objective To summarize the first aid and care of 28 severe hand - foot - and - mouth disease children with stress gastric ulcer receiving mechanical ventilation. Methods Nasal feeding was adopted in 62 severe hand - foot - and - mouth disease children with mechanical ventilation. Twenty - eight cases were confirmed with stress gastric ulcer by drainage of blood. Intravenous infusion of Omeprazole, nasal feeding thrombin, Smecta, admixture milk with saline solution, and gastric lavage with cold saline were administered. Results Stress ulcer was cured within 1 3 days. No hemafecia and upper gastrointestinal hemorrhea were observed. Conclusion Stress ulcer might be cured by intravenous infusion of Omeprazole, nasal feeding medicine and milk, and gastric lavage.
