摘 要:目的通过对顺德勒流地区上尿路结石的成分分析,并对草酸钙结石患者进行饮食危险因素的病例对照研究,探讨本地区尿路结石形成的病因学,为临床预防、治疗提供依据。方法回顾性分析2008—2010年156例上尿路结石患者的临床资料,并对结石标本进行化学成分测定对确诊为草酸钙结石患者进行病例对照研究,收集患者饮食危险因素。结果上尿路结石男性发病率高于女性,男女病例为2.32:1,好发年龄为25~65岁,占87.9%,结石中草酸钙检出率最高,单一成分全部为草酸钙结石,混合结石几乎含有草酸钙成分;经单因素条件Logistic回归分析,饮水量、喜饮酒、海鲜类、喜甜食及喜食蔬菜等因素与草酸钙结石的发病有统计学意差异;多因素条件Logistic回归显示不喜欢饮水、喜饮酒、喜食海鲜及甜食的回归系数都为正值,OR大于1,并且都有统计学意义。结论结石成分分析及结石饮食危险因素的病例对照研究对尿路结石的形成及预防方法具有重要意义。Objectives To investigate the etiology of formation of urinarytract stones in Shunde Leliu area, the analysis of chemical constituents was carried out for 156 casses upper urinary tract stones,and the patients with calciumoxalate stones were conducted by case - control study of dietary risk factor, which can provide the basis for prevention and treatment of urinary tract. Methods Cinical date of stone patients was analyzed from 2008 to 2010 periods, and stones samples was carried by chemical analysis ; (2)The dietary risk factors of patients were collected for case - control study. Results Upper urinary stones were more often found in males than in females, and the ratio was 2.32 : 1, especially in those 25 to 65 years old. The most common component of the urinary stones are calciumoxalate stones, mixed stones contains almost composition of calcium oxalate; single -factor conditional logistic regression displayed that water intake, drinking, seafood, sweets, vegetables and the occurrence of calcium oxalate have statistic differences. Multivariate conditional logistic regression showed that the regression coefficient of lack of water intake, seafood, sweets and drinking are positive, which have statistic differences similarly. Conclusions Chemical composition analysis and case - control study may provide important information to the etiology and prevention of urinary stones.
