中药及胸大肌带蒂肌肉瓣移植治疗胸骨后感染的临床研究

摘　　要：目的通过用中药及胸大肌带蒂肌肉瓣移植治疗术后胸骨感染,观察其效果。方法中药治疗胸大肌带蒂肌肉瓣移植在全麻下进行,用过氧化氢、聚维酮碘及生理盐水彻底冲洗伤口,缝合皮下不留死腔,皮下引流保持通畅,胸部加压包扎。结果 22例伤口一期愈合,8例伤口二期愈合,时间最短3周,一般约1个月,随诊6～24个月,平均8个月,伤口无炎性表现,呼吸正常,无其他并发症出现,收到较好的效果,达到了减少胸骨后感染的死亡率,提高了治愈率。结论中药及胸大肌带蒂肌肉瓣移植,治疗术后胸骨后感染是一种较好的治疗方法,在临床有推广使用价值。OBJECTIVE To observe the effect of transfering and chest big muscle pedicle muscle flap treat retrosternal infection and chinese medicine and observe its effect.METHODS Chinese medicine treatment,chest big muscle pedicle muscle flap transplanted under general anesthesia,hydrogen peroxide k90 iodine and physiological saline flushed the wound thoroughly,sutured subcutaneous no the dead space,kept subcutaneous drainage unobstruct,bandaged the chest by compression.RESULTS Totally 22 cases of phase 1 healing wounds,8 cases of phase 2 healing wounds,the shortest time was three weeks,general 1 months or so,follow-up 6 to 24 months,8 months in average,the wounds had not inflammatory performance,the breath was normal,no other complications,received good effect,reduced the mortality of the retrosternal infection,increased the cure rate.CONCLUSION Transplantation of Chinese medicine and chest big muscle pedicle muscle flap is a better treatment for retrosternal infection and is worthy of clinical promotion.
