摘　　要：目的探讨危重型孕产妇甲型H1N1流感的流行病学及临床特征，为该病的防制、诊断和治疗提供理论依据。方法对阜阳市第二人民医院所收治的11例危重型孕产妇甲型H1N1流感病例的相关资料进行回顾性分析。结果11例患者。年龄19-25岁，平均22．5岁。初产妇9例，经产妇2例，发病前7d内有发热流感样患者密切接触史。中孕1例，晚孕10例。病情危重期出现在病程第3～5d。临床表现为以急性呼吸窘迫综合征为主的多器官功能障碍。治疗上采取了适时终止妊娠，以抗流感病毒、机械通气及免疫球蛋白应用为主的综合性措施。治愈10例，死亡1例。结论甲型H1N1流感是一种新发的致命性传染病，中晚期孕妇危重型发生率高，病情进展迅速，临床表现为病毒性肺炎和急性呼吸窘迫综合征。早期抗病毒治疗、及时终止妊娠和适时机械通气是降低病死率的关键环节。Objective This paper discusses the epidemiology and clinical characteristics of critical type of maternal influenza A H1N1 influenza, in order to provide a theoretical basis for the control, diagnosis and treatment of this disease. Methods To retrospectively analyze 11 cases of critical type of maternal influenza A H1N1 influenza. Results 11 patients aged 19-25 years, mean 22.5 years old. Primipara 9 cases, multipara 2 cases, had close contact with patients with influenza-like fever 7 days before invasion. 2 cases of pregnancy, 9 cases of late pregnancy. Critical condition arise in the first 3-5 days. Clinical manifestations are the multiple organ dysfunction based on the acute respiratory distress syndrome. Take comprehensive measures which depend on anti-influenza virus, mechanical ventilation and the application of immuneglobulin after the timely termination of pregnancy. 9 cases were cured, 2 died. Conclusion Influenza A H 1N 1 influenza is a deadly emerging infectious disease. Pregnant women have high incidence of severe type and rapid progression. The clinical manifestations are viral pneumonia and acute respiratory distress syndrome. Early antiviral treatment, timely termination of pregnancy and timely mechanical ventilation is the key to lowering mortality.
