锁骨钩钢板治疗NeerⅡ型锁骨外侧端骨折30例疗效分析

摘　　要：目的探讨对NeerII型锁骨外侧端骨折进行切开复位锁骨钩钢板内固定手术的临床效果。方法2008年5月-2010年5月，对30例NeerII型锁骨远端骨折采用锁骨钩钛板进行切开复位内固定术，对临床资料进行回顾性分析总结。结果本组30例获得随访，随访时间6～24个月，采用Lazzcano标准评定息肩术后功能，优28例，良2例。未发现锁骨下血管、神经损伤，术后切口无感染。随访的x线片未见骨不连、肩关节半脱位、钢板断裂和松动等现象。术后10周均恢复日常生活和工作，有2例出现轻度肩关节外展撞击征，肩关节外展活动部分受限。结论对于NeerⅡ型锁骨外侧端骨折，锁骨钩钛板是一种较为安全、可靠的固定方法。Objective To investigate the clinical effect of Neer Ⅱ type of distal clavicle fractures with open reduction and internal fixation for clavicular hook plate. Methods May 2008 - May 2010, 30 cases of Neer Ⅱ distal clavicle fracture with clavicle hook plate for open reduction and internal fixation, the clinical data were retrospectively analyzed and summarized. Results Totally 30 patients were followed up for 6 to 24 months, using Lazzcano standard evaluation of postoperative shoulder function, excellent in 28 cases, good in 2 cases. Found no subclavian vessels, nerve injury, postoperative incision infection. No follow - up x - ray films of nonunion, shoulder subluxation, plate fracture and loosening phenomenon. After 10 weeks, were back to their daily life and work, 2 patients had mild impingement shoulder abduction, shoulder part of outreach activities is limited. Conclusions Type Neer Ⅱ fracture of distal clavicle, clavicular hook plate is a more safe and reliable fixation.
