超声刀在胸部创伤胸腔镜手术中的应用

摘　　要：目的探讨超声刀在胸部创伤胸腔镜手术中的应用效果。方法回顾性分析2005年8月～2010年7月经胸腔镜手术治疗93例胸部创伤患者的临床资料,手术使用超声刀。93例中进行性血胸19例,创伤性气胸15例,凝固性血胸53例,心包积液2例,创伤性膈疝4例。结果所有患者手术均成功完成,其中84例经胸腔镜手术,9例经胸腔镜辅助小切口手术。平均手术时间63分钟,术中平均出血量185ml,术后24小时平均引流液156ml,术后胸腔闭式引流管使用时间1～6天,平均2.9天,术后恢复良好,无术中及术后并发症。术后住院时间5～23天,平均12.3天。结论胸部创伤使用胸腔镜超声刀手术,术中出血少,手术时间短,超声刀是胸腔镜手术的良好辅助工具。Objective To discuss the effect of ultrasound knife in video-assisted thoracoscopic surgery for treating thoracic injury.Methods Data of 93 patients with thoracic injury who underwent video-assisted thoracoscopic surgery from Aug.2005 to Jul.2010 were reviewed.The ultrasound knife was applied in the surgery.All the 93 patients were diagnosed as massive hemothorax in 19,traumatic pneumothorax in 15,coagulated hemothorax in 53,pericardial effusion in 2,traumatic diaphragmatic hernia in 4.Results All operations for the 93 patients were performed successfully.A total of 84 patients were performed the video-assisted thoracoscopic surgery successfully and 9 patients were treated with video-assisted mini-incision surgery.The outcome of the operation revealed mean operation time of 63 minutes,mean blood loss of 185ml,and mean drainage of 156ml during 24 hours after operation.The chest drainage catheter was removed at the postoperative day 2.9（range,1-6 days）.All patients recovered well after surgery and had no intra-operative complications or postoperative complications.Hospitalization lasted 12.3 days on average（range 5-23 days）.Conclusion The application of ultrasound knife has the advantages of less blood and shorter operation time.Ultrasound knife is a good,useful tool for treating video-assisted thoracoscopic surgery.
