胰十二指肠切除术后胰胃吻合术与胰空肠吻合术手术并发症比较分析

摘　　要：目的比较分析胰胃吻合术与胰空肠吻合术在胰十二指肠切除术后并发症的发生情况。方法对2007年7月—2011年6月期间32例壶腹周围癌患者实施胰十二指肠切除,胰空肠吻合术和胰胃吻合术重建消化道的临床资料进行回顾性分析。结果胰胃吻合术后胰瘘,胆瘘,腹腔出血及死亡概率较胰空肠吻合术明显降低。而在吻合口出血,术后糖尿病及术后脂肪泻等并发症无差异。结论胰胃吻合术是胰十二指肠切除术后一种可降低胰瘘发生率的消化道重建方法。Objective To analyze the incidence of complications of pancreatogastrostomy with the pancreagicojejunostomy after pancreatoduodenectomy.Methods The data of 32 cases of periampullary tumours that implement pancreaticoduodenectomy,with pancreatogastrostomy and Pancreagicojejunostomy of digestive tract reconstruction during July 2007 to June 2011 were retrospectively analyzed.Results After pancreatogastrostomy,pancreatic fistula,biliary fistula,intra-abdominal bleeding and the probability of death than the pancreagicojejunostomy was significantly reduced.In the anastomotic bleeding,postoperative diabetes and complications such as postoperative steatorrhea was no difference.Conclusions Pancreatogastrostomy after pancreatoduodenectomy is one kind of tract reconstruction can reduce the incidence of pancreatic fistula.
