面部旋转皮瓣修复下睑皮肤恶性肿瘤根治术后创面

摘　　要：目的探讨面部旋转皮瓣修复下睑皮肤恶性肿瘤根治术后创面的临床效果。方法对13例皮肤恶性肿瘤根治术后遗留的下睑前层创面（1．0cm×0．8cm-4．0cm×3．0cm），采用局部面部旋转皮瓣推进修复。术中注意皮瓣深层适当固定，避免皮瓣对下眼睑的垂直牵引力，并在皮瓣旋转后引起局部皮肤隆起的同时，进行整形修复。结果所有患者术后平均随访1年，其皮瓣均存活，肿瘤无复发，下眼睑无外翻或退缩，眼睑闭合正常；皮瓣肤色、厚薄和质地与下睑匹配，手术瘢痕隐蔽细小，面部无明显畸形。结论应用面部旋转皮瓣修复下睑恶性皮肤肿瘤根治术遗留的较大面积前层创面，其方法简单、方便，疗效满意，尤其适合于中老年患者的治疗。Objective To explore the clinical results of defects repaired by rotated cheek flaps after excision of skin malignant tumors of the lower eyelid. Methods After excision of the skin tumors, the anterior lamella defect ranged from 1.0 cm×0.8 cm to 4.0 cm× 3.0 cm. All defects were reconstructed by rotation cheek flaps. It was important to anchor the deep layer of the flap to the periosteum of the orbital bone to prevent the vertical force on the lower eyelid. It needed to repair when arising local skin biopsy after flap rotation. Results All flaps survived totally. After mean 1 year following up, all cases were satisfied with the normal catacleisis, matched color, texture and thickness with concealed scar. There were no ectropion or retraction, facial deformity and recurrence occurring. Conclusion It is a simply and reliable method to repair the defects with rotated cheek flaps after excision of skin malignant tumors of the lower eyelid, particularly effective for the middle or old aged patients.
