后纵隔髓外造血组织瘤样增生的影像诊断

摘　　要：目的:探讨后纵隔髓外造血组织瘤样增生的影像表现。方法:结合文献,回顾性分析我院7例经病理证实的后纵隔髓外造血组织瘤样增生影像表现。结果:6例CT和MRI扫描表现为双侧中下段胸椎旁多发结节状软组织肿块,1例CT显示右后纵隔占位,肿块边缘光滑,密度均匀;MRI上呈等T1信号和等长T2信号,增强后明显均匀强化,增强曲线呈速升缓降型。结论:后纵隔髓外造血组织瘤样增生的影像表现有一定的特点,结合临床病史,可作出影像学诊断。Objective：To investigate imaging appearances of tumor-like hyperplasia of extramedullary hematopoietic tissue in the posterior mediastinum,including CT and MR imaging features.Methods：A retrospective analysis of CT and MR images was performed in 7 patients with pathologically proven tumor-like hyperplasia of extramedullary hematopoietic tissue in the posterior mediastinums.Results：Of the 7 patients,6 had bilateral,multiple paravertebral soft tissue masses located broad-based at the mid-and lower thoracic vertebrae on CT and MRI,one case with well-delineated,homogenous soft tissue mass in right posterior mediastinum.MRI appeared isointense on T1WI and iso-/hyper-intense on T2WI.The enhancement pattern of tumor-like hyperplasia of extramedullary hematopoietic tissue on multiphase dynamic MRI was fast wash-in/slow wash-out.Conclusion：Tumor-like hyperplasia of extramedullary hematopoietic tissue in the posterior mediastinum has some specific imaging appearances.Imaging examination combined with clinical manifestations is helpful for accurate diagnosis of tumor-like hyperplasia in extramedullary hematopoietic tissue in the posterior mediastinum.
