摘 要:目的:观察育龄期妇女妊娠中、晚期血清叶酸、VitB12、铁蛋白、血红蛋白及血小板浓度的变化,探讨血清叶酸、VitB12、铁蛋白浓度与妊娠期并发症及结局的关系。方法:选择2009年9月—2010年6月在本院就诊的孕产妇571例,选择同期健康育龄非孕妇女200例为对照组,检测2组血清叶酸、VitB12、铁蛋白、血红蛋白及血小板浓度变化,观察妊娠并发症发生及结局的情况。结果:(1)妊娠中、晚期血清叶酸、VitB12及铁蛋白低浓度发生的比例随孕周增长而增多,VitB12及铁蛋白平均浓度随孕周增长而降低(P<0.05),妊娠期随孕周增长发生贫血的比例显著增高,血红蛋白平均浓度显著下降,有显著差异(P<0.05),血小板浓度无显著变化(P>0.05);(2)孕中、晚期血清叶酸低浓度与发生贫血、妊娠期高血压疾病相关,孕中期血清叶酸高浓度与发生妊娠期糖尿病相关(P<0.05),孕晚期血清VitB12低浓度与发生羊水过多相关(P<0.05),孕晚期血清铁蛋白低浓度与发生贫血相关(P<0.05)。结论:妊娠中、晚期血清叶酸、VitB12及铁蛋白变化与妊娠期多种并发症相关,因此定期检测可评估孕妇及胎儿营养代谢状况,对指导孕妇合理膳食及诊疗妊娠期相关并发症具有重要的优生学意义。Objective:To observe the change of serum levels of folate,vitamin B12,ferritin,hemogloblin and platelet concentration levels of pregnant women in second and third trimester of pregnancy of reproductive age,and to investigate the relationship between the concentration of serum folate,vitamin B12,ferritin and various complications and their outcomes during pregnancy.Methods:We selected 571 gravidas who visited in our hospital from September 2009 to June 2010,and 200 healthy non-pregnant women of reproductive age as a control group.We detected the change of serum folate,vitamin B12,ferritin,hemogloblin and platelet concentration levels,and observed the pregnant complications and outcomes.Results: In pregnancy,low concentration of serum folate,vitamin B12 and ferritin levels occurred with increasing gestational age,the concentration of ferrin and vitamin B12 was significantly different with increasing gestational age(P0.05).The proportion of anemia was high with the gestational age increasing,the average hemoglobin concentration was significantly different with increasing gestational age(P0.05).Anemia,hypertensive disorders complicating pregnancy were related to low serum folate concentrations in second and third trimester,while high concentration in second trimester was related to gestational diabetes mellitus(P0.05).There was relationship of low concentration of vitamin B12 and polyhydramnios(P0.05).Also low concentration of the serum ferritin was related to anemia(P0.05).Conclusions: The change of serum folate,vitamin B12 and ferritin concentration levels in second and third trimester are related to various pregnant complications and outcomes.Therefore,in pregnancy regular testing those levels are of great clinical significance on eugenics to understand and assess the status of maternal nutrition and metabolism,to provide an objective basis to guide a reasonable diet of pregnant women.Meanwhile,it has some significance of diagnostics,prevention and treatment of the pregnant complication
