胰岛素抵抗与脂代谢异常在HDCP发病中的作用及相关性

摘　　要：目的:研究胰岛素抵抗与脂代谢异常在妊娠期高血压疾病(HDCP)发病中的作用及相关性。方法:以62例HDCP患者为研究对象,并以同期正常孕妇32例为对照组。检测两组空腹血糖(FBG)、胰岛素(FINS)、总胆固醇(TC)、甘油三酯(TG)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL)、载脂蛋白A-1(apoA-1)、载脂蛋白B(apoB)的含量,以HOMA指数为胰岛素抵抗指标,分析以上各指标在两组孕妇中的差异及其相关性。结果:HDCP组与正常妊娠组相比,其HOMA指数、TG、LDL、apoB显著升高,且随病情的加重逐渐升高;HDL、apoA-1显著降低,且随病情加重而逐渐降低。HOMA指数与脂代谢各指标间具有显著相关性。结论:HDCP患者存在胰岛素抵抗与脂代谢异常,并且两者具有相关性。Objective：To investigate the correlation between insulin resistance and abnormal lipid metabolism in patients with hypertensive disorder complicating pregnancy（HDCP）. Methods：62 patients with HDCP and 32 normal pregnant women as controls were enrolled in this study.Serum concentration of fasting blood glucose（FBG）,fasting insulin（FINS）,total cholesterol（TC）,triglyceride（TG）,high density lipoprotein（HDL）,low density lipoprotein（LDL）,apolipoproteinA-1（apoA-1）,and apolipoprotein B（apoB） were measured.Insulin resistance was estimated by homeostasis model assessment（HOMA）：HOMA=FBG×FINS/22.5.The difference of these agents in normal pregnant women and patients with HDCP was studied.The association of insulin resistance with abnormal lipid metabolism in patients with HDCP was also studied. Results：Compared with controls group the levels of HOMA,TG,LDL,apoB were significantly higher;the levels of HDL,apoA-1 were significantly lower.Correlation was shown among HOMA and lipid metabolism. Conclusion：Patients with HDCP has insulin resistance and abnormal lipid metabolism and there was correlation between both sides.
