凝固酶阴性葡萄球菌抗药基因检测及其对消毒剂抗性研究

摘　　要：目的研究医院分离的凝固酶阴性葡萄球菌抗药基因携带情况及其对消毒剂的抗性关系。方法采用PCR方法和肉汤稀释法，分别对凝固酶阴性葡萄球菌qacA基因及其对消毒剂抗性进行了检测。结果从医院环境和医务人员手上分离的63株凝固酶阴性葡萄球菌中，检出12株携带qacA基因，检出率为19．05％。聚维酮碘对12株携带抗药基因的菌株MIC值在250～500mg／L之间，对其中3株菌的MBC为1000mg／L，超过标准菌株的500mg／L。12株携带抗药基因的凝固酶阴性葡萄球菌中，二氯异氰尿酸对其中3株菌的MIC值为500mg／L、对其中5株菌的MBC值为500mg／L；三氯异氰尿酸对其中7株菌的MIC值介于500—1000mg／L之间、有3株MBC值为1000mg／L，均超过标准菌株的250mg／L。结论凝固酶阴性葡萄球菌抗药基因携带率为19．05％，携带qacA基因的凝固酶阴性葡萄球菌部分菌株分别对含碘消毒剂和含氯消毒剂的抗性有所增加。Objective To study the relation between the resistance genes carrying condition of the coagulase - negative staphylococcus and its disinfectant resistance. Methods PCR and broth dilution methods were used to detect the qacA gene and the disinfectant resistance of coagulase - negative staphylococcus. Results 12 strains carrying qacA gene were detected from the 63 coagulase - negative staphylococcus separated from the hospital environment and hands of medical staff and the detection rate was 19.05%. The MIC values of povidone iodine for 12 strains carrying drug - resistant genes were between 250 - 500 mg/L. The MBC values of 3 strains carrying drug - resistant genes were 1 000 mg/L which above the standard value 500 mg/L. The MIC values of dichloroisocyanuric acid for 3 strains coagulase - negative staphylococcus car- rying resistance gene were 500 mg/L and for 5 strains of which the MBC values were 500 mg/L, for 7 strains of which the MIC values of tricloro - isocyanuric acid were between 500 mg/L and 1 000 mg/L, for 3 strains of which the MBC values were 1 000 mg/L. All of these were above the standard value 250 mg/L. Conclusion The resistance gene carrying rate of coagulase - negative staphylococcus is 19.05%. The disinfectant resistance of some strains of coagulase - negative staphylo- coccus carrying qacA gene to Iodine disinfectant and chlorine disinfectants have increased.
