前路病灶清除并后路椎弓根系统固定术治疗多节段胸腰椎结核48例临床分析

摘　　要：目的:探讨多节段胸腰椎结核采用前路病灶清除并后路椎弓根系统固定术治疗的临床效果。方法:回顾性分析2008年1月～2010年6月我院采用前路病灶清除结合后路椎弓根系统内固定术治疗的48例多节段胸腰椎结核患者的临床资料。结果:本组患者术后切口均为Ⅰ期愈合。1年后随访,所有患者腰背疼痛症状消失,无局部结核复发,植骨全部骨性融合,Cobb角矫正至5°～19°,平均12°。脊髓神经功能按ASIA分级:恢复至D级5例,恢复至E级43例。结论:前路病灶清除结合后路椎弓根系统内固定术治疗多节段胸腰椎结核疗效可靠、安全、恢复快,值得推广应用。Objective： To explore the clinical effects of anterior radical debridement and posterior spine system fixation in the treatment of multiple segment thoracolumbar tuberculosis.Methods： Clinical datas of 48 patientes of multiple segment thoracolumbar tuberculosis with the treatment of anterior radical debridement and posterior spine system fixation from January 2008 to June 2010 in our hospital were retrospectively analyzed.Results： Postoperation incision of all patients were healed with stage Ⅰ.The waist back pain symptoms of all patients disappeared at the follow-up visit after 1 year,no local recurrence of tuberculosis,all bone graft with bony fusion,Cobb angle was corrected to 5°-l9°,the average level of which was l2°.Spinal neurological was divided according to the ASIA.5 cases received level D,43 cases in level E.Conclusion： Effect of multiple segment thoracolumbar tuberculosis treatment with anterior radical debridement and posterior spine system fixation is reliable,safe and rapidly recoverable,and is deserved to be widely used.
